Suncoast Hospice Foundation is a Palm Harbor, Florida-based nonprofit that raises funds for Empath Health and its family of care programs, including Suncoast Hospice. Since 1982, the foundation has helped fund hospice, grief, senior, home health, palliative, and patient-support services for individuals and families across Pinellas County and nearby Florida communities.
Suncoast Hospice Foundation financially backs the care delivered through Empath Health members, including Suncoast Hospice, Suncoast PACE, Empath Home Health, EPIC, Empath Grief Care, and palliative care services. These programs serve people of all ages facing chronic illness, advanced illness, end-of-life needs, HIV, grief, caregiving challenges, and complex health conditions.
Suncoast Hospice has provided end-of-life care for Pinellas County residents since 1977. Its care includes medical, palliative, personal, spiritual, emotional, social, volunteer, and grief support wherever patients call home.
Suncoast PACE provides all-inclusive care for seniors age 55 and older with chronic health conditions, including primary care, therapies, adult day services, in-home care, caregiver education, and other supports that help participants remain in their homes.
The foundation’s giving opportunities also fund services not always reimbursed by Medicare, Medicaid, private insurance, or other coverage. Donor-funded examples listed by the foundation include bereavement sessions, pet care for hospice patients, a day of hospice care, grief support for children and parents, and meal assistance for people affected by HIV.
